Tyler ISD will accept substitute applications until
Thursday, October 15th.
Substitute training will be held on Friday, October 30, in the Davidson
Conference Center at the Plyler Instructional Complex, from 9:00 a.m. to
12:00 p.m. In order to be considered for a substitute
position, you will need to complete the following forms:
Substitute Application for TEACHERS
Substitute
Application for PARAPROFESSIONALS
Federal Withholding Exemption Form (W-4)
Criminal History Record Information
You may download and print these forms or pick up paper copies at the Central
Administration office. These forms are in PDF format and require the free program Adobe Acrobat Reader
to download.
Remember that you will also need to bring the following
when filling out a substitute teaching application:
A copy of your college transcript Original proof of identity and a copy of those documents for
our records (driver's license and social security card are acceptable.) A copy of your teaching certificate (if applicable).
