Head
Start is a federally funded program that services primarily
low-income children and their families. The families in Head
Start must qualify according to the income guideline set by the
federal government and be age appropriate. Please call

to Stewart Middle School).
The
child must be 4 Years Old by Sept. 1, 2008. Children who are 3
yrs old with a
history of a diagnosed
disability (that meets school standards) will be considered for enrollment.
Requirements:
Please bring the following information.
1. Proof of
Income (Tax Return for 2007)
2.
Certified Birth Certificate
3.
Immunization Record
4.
Social Security Cards for all family members
5.
Proof of Address
(utility bill [gas, water, electricity] or rent
receipt)
6. Proof
of Insurance / Medicaid / CHIP
(if applicable)
